One documented instance concerns Iran's nuclear program. Broadcast coverage repeated the concern while dropping the "no evidence of a bomb" qualifier entirely.

April 6, 2026: The American Nuclear Society reported IAEA Director General Rafael Grossi's statement that there was no evidence of Iran building a nuclear bomb, while noting Iran's large stockpile of near-weapons-grade enriched uranium and its refusal to grant full inspector access were causes for serious concern.
